About modification and amendments in some legal acts

Art. I. - In the Code of civil procedure of the Republic of Moldova No. 225-XV of May 30, 2003 (Official monitor of the Republic of Moldova, 2003, No. 111-115, of the Art. 451), with subsequent changes, to make the following changes and addition:

"Article 286. Filing of application

(2) the Adoptive father - the citizen of the Republic of Moldova living abroad and the adoptive father - the foreign citizen or the stateless person, the person interested to adopt the child living in the Republic of Moldova submits directly or through the accredited foreign organization with powers in the field of the international adoption the statement for establishment of the international adoption to court at the place of residence of the adopted child.

Article 287. Contents of the statement

The statement for establishment of adoption shall contain:

a) surname and name, year, month and number of the birth of the adoptive father, its residence;

b) surname and name, year, month and number of the birth of the adopted child, its residence;

c) request for change of surname and/or name of the child; if adoptive parents have different surnames - specifying of surname which will be carried by the child; request for change of the birthplace of the child;

d) certificate number of the adoptive father, date of issue and the body which issued it - in case of national adoption;

e) another supplied and the information upon the demand of degree of jurisdiction.
