In forms of the statements for confirmation of the All-Ukrainian status of public association and of refusal of the All-Ukrainian status of public association approved by the order of the Ministry of Justice of Ukraine of December 14, 2012 No. 1842/5, registered in the Ministry of Justice of Ukraine on December 18, 2012 for word No. 2100/22412," (the name of authorized body concerning registration on the location of public association)" shall be replaced with words "The public registration service of Ukraine".
2. To department of interaction with authorities (Zerkal O. V.) to submit this order on state registration according to the Presidential decree of Ukraine of October 03, 1992 to No. 493 "About state registration of regulatory legal acts of the ministries and other executive bodies".
3. This order becomes effective from the date of its official publication.
4. To impose control over the implementation of this order on the director of the department of interaction with authorities Zerkal O. V.
