About modification of Rules of state registration of acts of civil status in Ukraine
According to article 13 of the Law of Ukraine "About state registration of acts of civil status" and the Procedure for confirmation of the fact of the birth of the child out of health care institution approved by the resolution of the Cabinet of Ministers of Ukraine of January 09, 2013 No. 9, and also for the purpose of reduction of regulatory legal acts of the Ministry of Justice of Ukraine in compliance with requirements of the legislation I ORDER:
1. Bring in the Rules of state registration of acts of civil status in Ukraine approved by the order of the Ministry of Justice of Ukraine of October 18, 2000 No. 52/5, registered in the Ministry of Justice of Ukraine on October 18, 2000 for No. 719/4940 (in edition of the order of the Ministry of Justice of Ukraine of December 24, 2010 No. 3307/5), such change:
ІІІ to state the second offer of the subitem "an" of Item 2 of Chapter 1 of the Section in such edition: "In case of the child's birth out of health care institution state registration of the birth is carried out based on the health certificate on the birth or the medical certificate about stay of the child under the supervision of medical institution (form No. 103-1/o) which form is approved by the order of the Ministry of Health of Ukraine of August 08, 2006 No. 545, registered in the Ministry of Justice of Ukraine on October 25, 2006 for No. 1150/13024 (with changes) (further - the medical certificate about stay of the child under the supervision of medical institution), and the conclusions about confirmation of the fact of the birth of the child out of health care institution in the form established in appendix 3 to the Procedure for confirmation of the fact of the birth of the child out of health care institution approved by the resolution of the Cabinet of Ministers of Ukraine of January 09, 2013 No. 9".
