1. These Methodical recommendations determine single requirements for filling of report form about work of local executive body for internal control behind quality of the provided state services (further - the Report form) and have advisory nature.

2. The report form is developed in compliance by the subitem 18) of Item 15 of the Regulations on the Agency of the Republic of Kazakhstan for public service approved by the Presidential decree of the Republic of Kazakhstan of December 3, 1999 No. 280. The main objective of introduction of this form is carrying out monitoring of results of internal control of quality of rendering the state services.

3. The report form is filled and provided by akimat of area, the cities of Astana and Almaty taking into account local executive bodies of area, the cities of republican value, the capital, areas, the cities of regional value, akims of areas in the city, the cities of district value, settlements, villages, rural districts every month to the 5th following the accounting period.

4. The report form is filled and provided with the accruing result based on data of primary accounting organized concerning the internal control behind quality of the provided services on the first following the accounting period.
