1. Bring in the Form of the registration certificate on medicinal means / medicine (medical immunobiological supply) approved by the order of the Ministry of Health of Ukraine of July 29, 2003 No. 358, registered in the Ministry of Justice of Ukraine on August 07, 2003 for No. 693/8014, such changes:

the fourth after the word "Applicant" to add the paragraph with the words "and its location";

in paragraph six to replace the word "issued" with the word "drawn up".

2. Bring in the Description of the registration certificate on the medicinal means / medicine (medical immunobiological supply) approved by the order of the Ministry of Health of Ukraine of July 29, 2003 No. 358, registered in the Ministry of Justice of Ukraine on August 07, 2003 for No. 693/8014, such changes:

2.1. In Item 3:

to add the paragraph of the sixth after the word "Applicant" with the words "and its location";

in paragraph eight to replace the word "issued" with the word "drawn up".
