"8. When passing accreditation for carrying out independent expert evaluation of activities of subjects of health care physical persons submit the following documents to accrediting body:

1) the statement on passing of accreditation in the form approved by accrediting body with indication of the declared area of accreditation according to the nomenclature of specialties approved by authorized body;

2) copy of the identity certificate;

3) the copy of the diploma about the higher professional medical or pharmaceutical education;

4) the copy of the service record or other document confirming availability of length of service according to subitems 1), 2) or 3) of Item 8-1 of these rules;

5) copies of documents on advanced training and (or) retrainings taking into account requirements of parts two, third Item 8-1 of these rules (for the specialists performing clinical practice and pharmaceutical activities);

6) copies of certificates on assignment of the first or highest qualification categories (for the specialists performing clinical practice);

7) copies of documents on availability of academic degrees, ranks (in case of their availability).";
