1. Make to the section IV of Rules of state registration of the acts of civil status in Ukraine approved by the order of the Ministry of Justice of Ukraine of October 18, 2000 No. 52/5, registered in the Ministry of Justice of Ukraine on October 18, 2000 at No. 719/4940 (in edition of the order of the Ministry of Justice of Ukraine of December 24, 2010 No. 3307/5), the following changes:

2) after Item 1 to add the Section with new Items 2, 3 following of content:

"2. If the original certificate about state registration of the act of civil status is stolen, lost, damaged or destroyed, repeated issue of the certificate is performed by departments of state registration of acts of civil status based on assembly record of the civil status constituted both in electronic form, and on paper.

Repeated issue of certificates on state registration of acts of civil status is in that case performed by departments of state registration of acts of civil status irrespective of the place of state registration of the act of civil status and the place of residence of the applicant.
