2. In the Card of reception of an application approved by the order of the Ministry of Justice of Ukraine of April 17, 2012 No. 595/5, registered in the Ministry of Justice of Ukraine on April 20, 2012 on word No. 590/20903, "the notary the name of body of state registration of the rights or the name of notary office, the name of the notarial district" shall be replaced with words "person adopting the statement".
