Conditions of provision of the state service

Service provision to visitors is performed:

- in the rooms answering to the established sanitary standards;

- on the first-come, first-served basis;

- in the presence of easy access of citizens to sanitary and hygienic rooms (toilets, washing rooms).

Carrying out initial consulting of the consumer of state service by specialists by phone is possible.

Preferential categories of citizens (participants and disabled people of the Great Patriotic War, home front workers, LOVZ, expectant mothers) are serviced out of turn.

For convenience of visitors in the place of provision of service the list of the documents necessary for service acquisition, and samples of requests are placed

Term of provision of the state service

Confirmation of competence of laboratory by authorized body in the field of accreditation includes the following stages and terms:

- acceptance of the request with verification of the list of documents - to 5,5 of people/hours;

- duration of calculation of cost, the statement of the order/account on work on accreditations and execution of the agreement - till 5 people/hours;

- duration of creation of expert group with approval of concerned parties - till 4 people/hours;

- duration of conducting examination of documents - up to 60 working days, with submission of the concluding observation;

- duration of conducting repeated examination - up to 30 working days, with submission of the repeated concluding observation;

- evaluating duration on site with submission of the final report - to 15 days;

- duration of creation and work of the Commission on decision making and decision making - up to 15 days;

- duration of execution of the certificate of accreditation with approval of area of accreditation - till 2 people/hours;

- issue of the certificate of accreditation/refusal in accreditation - within 1 hour;

- duration of execution of the contract for carrying out inspection check-up behind activities of accredited laboratories and cards of inspection check-up - till 2 people/hours.

After receipt of the certificate of accreditation which effective period is determined 4 years according to the approved plan, twice for the specified period are performed activities of accredited laboratory, regarding observance of the established requirements of the international standard ISO/MEK 17025.

In case of receipt of claims or other negative data on activities of accredited laboratory for the decision of authorized body in the field of accreditation and confirmation of competence of conformity assessment bodies unplanned inspection check-up is carried out;

- duration of preparation of the notification on carrying out the inspection check-up behind activities of accredited laboratory approved with members of expert group - till 4 people/hours

Communication with visitors

In authorized state body in the field of accreditation and confirmation of competence of conformity assessment bodies information plates on doors of offices of the employees involved in service provision are placed. All employees working with the population have the personified plates (badges) with indication of surname, name, middle name and position.

In case of communication with visitors employees shall observe the following basic principles of ethics: be polite, benevolent, correct, patient, basic, aim to understand deeply being of question, be able to listen to the interlocutor and understand its line item, and also weigh and reason for the made decisions.

The job descriptions (functional obligations) and the professional ethical standards which are not allowing violations of the law of the Kyrgyz Republic, providing the ethics, independence and objectivity in relation to citizens excluding conflict of interest shall be observed by all employees.

Communication with persons with special needs on medical and social indications, and also provision of service are performed by it in form, clear and available to them

The list of necessary documents and/or actions from the consumer of the state service

Necessary documents for accreditation in case of confirmation of competence of laboratory:

a) the request of the established sample (it is posted on the website).

Note: in the request it is necessary to specify the address, phone number, the fax, e-mail, bank details, the RCBO code, the website of the organization applicant;

b) appendix to the request:

- copies of the charter and/or other constituent documents of the organization which part the laboratory or laboratories, with submission of the copy of the certificate on state registration in judicial authority is;

- the project of the declared area of accreditation (in the paper and electronic version);

- regulations on laboratory with appendix of organizational structure;

- quality manual and corresponding procedures;

- the passport of laboratory with the corresponding forms according to activities of laboratory (in the paper and electronic version);

- information on participation in interlaboratory checkings and control testing / calibrations in case of assessment on site or inspection check-up;

- plan of participation of laboratory in programs of skill test for quality of testing/calibrations, interlaboratory collation researches / calibrations;

- methods/procedure of internal calibration (in the presence);

- the price list of rates approved in the procedure established by the legislation.

c) procedure for submission of the request by the applicant with set of documents:

- application of the established sample with set of documents;

- agreement signature on work on accreditation of laboratory;

- payment for work on accreditation of laboratory during 5 banking days from the date of receipt of the invoice.

Actions of the applicant after receipt of the concluding observation on the carried-out expertize of documents:

- completion of materials - up to 60 working days;

- completion of materials following the results of repeated examination - up to 30 working days.

In the presence of written reasons for the customer the term of elimination of discrepancies by results of examination is prolonged up to 30 working days;

- acceptance by the applicant of expert group for evaluating on site - up to 3 working days;

- signing of the final report by the applicant - within 2 hours;

- development by the applicant of corrective actions from laboratory with discrepancies - up to 15 days;

- production of evidence by the applicant about elimination of discrepancies on the performed corrective actions - within 40 working days;

- the term of elimination of the discrepancies revealed in case of assessment of laboratory on site - up to 30 working days (in case of the written address of the applicant).

Actions of the applicant after receipt of the certificate of accreditation:

- agreement signature on carrying out inspection check-up of activities of accredited laboratory according to the card of inspection check-up;

- receipt of the notification and document for payment for carrying out inspection check-up;

- signing of the final report following the results of carrying out inspection check-up

expert group;

- development of corrective actions from laboratory with discrepancies within 5 days;

- production of evidence about elimination of discrepancies on the performed corrective actions from laboratory within 30 working days;

- receipt of the order on compliance of accredited laboratory to requirements of the international standard;

- receipt of the order on suspension of action of the certificate of accreditation with discrepancies;

- in case of accomplishment of corrective actions and production of evidence about elimination of discrepancies - receipt of the order on renewal of action of the certificate of accreditation and continuation of compliance to requirements of the international standard;

- in case of failure to carry out of corrective actions and non-presentation of proofs about elimination of discrepancies - receipt of refusal in accreditation

Cost of paid state service

The cost of the state service in accreditation of laboratory is established according to the price list of rates for the services in confirmation of competence of conformity assessment bodies which are carried out by the authorized body in the field of accreditation and confirmation of competence of conformity assessment bodies approved by the order of the Ministry of Economics of the Kyrgyz Republic in coordination with authorized state body in the field of the antimonopoly policy.

Parameters of quality of the state service

Quality of provision of the state service is determined by the following criteria:

- relevance for receivers of this state service;

- reliability and timeliness in accordance with the terms and the service provision terms declared in the standard of the rendered service;

- timeliness according to terms of provision of the state service (depending on the declared amount of works and terms of carrying out this state service);

- availability, reclamation at applicants only of those documents for receipt of service which are specified in the standard;

- compliance of conditions of provision of service to the requirements established by this standard: access to the building, availability of household conveniences, the schedule of acceptance convenient for citizens, availability and availability of information support (in printing and electronic formats);
