About introduction of amendments to the order of the Ministry of social policy of Ukraine of March 5, 2012 No. 122

According to the Procedure of activities for adoption and implementation of supervision of observance of the rights of the adopted children, No. 905 approved by the resolution of the Cabinet of Ministers of Ukraine of October 08, 2008 (with changes), and to Item 8 of the Regulations on the Ministry of social policy of Ukraine approved by the Presidential decree of Ukraine of April 6, 2011 No. 389 (with changes), I order:

1. Bring in the order of the Ministry of social policy of Ukraine of March 05, 2012 No. 122 "About approval of document forms concerning adoption", registered in the Ministry of Justice of Ukraine on March 23, 2012 on No. 441/20754, the following changes:

add the order after Item 2 with new Item of the following content:

"3. Approve forms of statements for adoption cancellation which are applied.".

With respect thereto to consider Items 3-6 respectively Items 4-7;

form of the Act of acquaintance of applicants with procedure and conditions of adoption, the rights and obligations of candidates for adoptive parents, adoptive parents, adoption consequence in law, procedure of control of conditions of accommodation and education of the adopted child to be reworded as follows which is applied.

2. And adoptions (P.C. Sausage) in accordance with the established procedure to provide to department of protection of the rights of children submission of this order on state registration in the Ministry of Justice of Ukraine.

3. To impose control over the implementation of this order on the deputy minister Fedorovich N. V.

4. This order becomes effective from the date of its official publication.
