About approval of the application form about exemption of the income tax of business entities based on h (17) Art. 24 of the Law on enforcement of Sections I and II of the Tax code No. 1164-XIII of 24.04.1997.

In pursuance of provisions of h (17) Art. 24 of the Law on enforcement of Sections I and II of the Tax code N 1164-XIII of April 24, 1997 (further - the Law) in edition of the Law on modification and amendments in some legal acts N 427-XV of December 24, 2004. (The official monitor of the Republic of Moldova N 1-4 of January 1-7, 2005), I ORDER:

1. Approve the application form about exemption of the income tax of the business entities specified in h (17) Art. 24 of the Law (Appendix N 1).

2. Oblige the territorial tax authorities to record statements for exemption of the income tax provided by the business entities specified in h (17) Art. 24 of the Law in the special register which will contain the following headings: sequence number; name of business entity; fiscal code of business entity; type of activity (according to the Appendix N 1 to h (17) Art. 24 of the Law); number and date of submission of the statement; room and registration date of the statement in tax authority; the period for which exemption of the income tax is requested.
