Make change to Item 20 of the Procedure for maintaining the State register of acts of civil status of citizens approved by the resolution of the Cabinet of Ministers of Ukraine of August 22, 2007 No. 1064 (The Official Bulletin of Ukraine, 2007, No. 65, Art. 2516; 2008, No. 21, Art. 600; 2011, No. 84, Art. 3078; 2012, No. 71, Art. 2870, No. 90, Art. 3651; 2015, No. 50, to the Art. 1601, No. 69, the Art. 2276, No. 102, the Art. 3525), having stated to paragraph one in the following edition:

"20. The statement from the Register is issued on the clean sheet of paper of the A4 format (210 x 297 millimeters) without use of the special form, makes sure the signature of the official and is sealed the relevant department of state registration of acts of civil status. In case, stipulated in Item 19-1 this Procedure, extraction from the Register is issued without certificate by the signature and fastenings seal.".

2. Determine that the available remaining balance of special forms is used by bodies of state registration of acts of civil status in full.
