About approval of the List of the documents attached to the request in case of certification of services in providing civil aircrafts with aviation fuel and special liquids for modification and (or) amendments in the certificate or receipts of the duplicate and the Program of certified inspection of services in providing civil aircrafts with aviation fuel and special liquids

According to the order of the Government of the Republic of Kazakhstan of November 11, 2004 No. 1187 "About approval of Rules of certification of services in the field of civil aviation",

I ORDER:

1. Approve enclosed:

1) the List of the documents attached to the request in case of certification of services in providing civil aircrafts with aviation fuel and special liquids;

2) Program of certified inspection of services in providing civil aircrafts with aviation fuel and special liquids.

2. To impose control of execution of this order on the vice-chairman of Committee of civil aviation of the Ministry of Transport and Communications of the Republic of Kazakhstan Adimold R. O.

3. This order becomes effective from the moment of its official publication.

The list of the documents attached to the request in case of certification of services in providing aircrafts of civil aviation with aviation fuel and special liquids for modification and (or) amendments in the certificate or receipts of the duplicate

1. The documents attached to the request for receipt of the Certificate of services in providing civil aircrafts with aviation fuel and special liquids:

1) the request in form according to the state system of certification of the Republic of Kazakhstan;

2) constituent documents of the applicant (the copy of articles of organization of civil aviation as legally independent company or the statement from the charter of the company which part the certified service on providing civil aircrafts with aviation fuel is);

3) the certificate on state registration of the legal entity, for physical person - the copy of the identity certificate or the passport, and the copy of the document granting the right to implementation of business activity without formation of legal entity;

4) the act on right to use by the parcel of land;

5) the insurance policies confirming obligatory types of insurance;

6) conclusions of state bodies of sanitary and ecological inspection;

7) the list of the measuring instruments which are subject to the obligatory metrological check approved with body for metrology;

8) regulations on service of fuels and lubricants;

9) the passport of the organization for aviation fuel supply including:

the name of aviation fuels and lubricants and special liquids with which the organization works;

list of the main production rooms;

list of processing equipment of system of aviation fuel supply;

list of technological gages of system of aviation fuel supply; staff list;

aggregated data on the leading and aviation personnel of system of aviation fuel supply;

10) process chart of work of service of fuels and lubricants;

11) scheme of transaction of system of aviation fuel supply and ensuring control;

12) technology of interaction of service of fuels and lubricants with services of the airport;

13) the characteristic of the existing system of aviation fuel supply;

14) the scheme of quality control of aviation fuel from acceptance from the petroleum tank vehicle before issue in the aircraft;

15) quality manual of fuels and lubricants;

16) the order on the admission of staff to work with special liquids during the autumn and winter and spring and summer periods;

17) the order on the admission to work on management of entrance and departure of special vehicles;

18) the list of the normative and leading documentation on aviation fuel supply of general nature;

19) schedule of the state check of measuring instruments;

20) the actions plan on advanced training of employees of service of fuels and lubricants;

21) structure of service of fuels and lubricants;

22) the list of instructions about measures of fire safety;

23) list of instructions for labor protection and safe engineering;

24) list of job descriptions;

25) testimonial of laboratory of service of fuels and lubricants;

26) technological scheme of warehouse of service of fuels and lubricants and management of latches.

2. The documents submitted by the applicant on modification and (or) amendments to the Certificate on providing civil aircrafts with aviation fuel and special liquids include:

1) the statement of any form with necessary reasons;

2) the original of earlier issued certificate.

3. For receipt of the duplicate of the Certificate the applicant files in authorized body petition of any form.

4. The documents specified in subitems of 2)-5) of Item 1 of this List are presented in the form of notarized copies.

The documents specified in subitems of 6)-26) of Item 1 of this List are presented in the form of the copies certified by the first head and the applicant's seal.

5. In case of submission of the request (statement) the submitted documentation is completed in any, convenient for consideration and use of the sequence. At the same time on the first page the list of documentation with indication of numbers of pages is specified.

All materials included in structure of the submitted documentation are drawn up in duplicate (on one for authorized body and the applicant).

Documents with the erasures, corrections which are not certified by the sign and seal are not taken cognizance.
