1. Approve the enclosed statement declaration form according to appendix to this order.

2. To provide to management on technical regulation and metrology this order on registration to the Ministry of Justice of the Republic of Kazakhstan.

3. To impose control of execution of this order on the vice-chairman Momyshev Talgat Amangeldinovich.

4. This order becomes effective from the date of official publication.
