1. "6) the standard of the state Service "Registration of the Children Who Are Citizens of the Republic of Kazakhstan, Transferred to Adoption (Adoption) to Foreigners" according to appendix 6 to this order;";

the certificate of birth (in case of receipt of the passport by the child which did not reach 16-year age);

the passport of the citizen of the Republic of Kazakhstan (in case of exchange of the passport);

four color photos the size 3, 5kh 4,5 of centimeters (further - cm) (photos shall correspond to age of uslugopoluchatel at the time of execution of the document and are executed strictly in fullface on light background with neutral look and the closed mouth in which person occupies about 75% of total area of the photo. Photos are carried out by photo printing method from one negative on dense photographic paper. Use of the images made by method of computer scanning, modeling or photocopying is not allowed);

the passport of sample of 1974 (the former USSR) - when obtaining for the first time passports the citizen of the Republic of Kazakhstan (in case of confirmation of nationality of the Republic of Kazakhstan);

In case of provision by uslugopoluchatel of incomplete document package according to the list, stipulated in Item 9 these standards of the state service, and (or) documents with the expired effective period the service provider refuses reception of an application, at the same time the service provider issues the receipt on refusal in documents acceptance.";

"10. Appeal of decisions, actions (bezdeystviye) of the service provider and (or) his officials concerning rendering the state services: the claim is submitted addressed to the head of the service provider to the address specified in Item 12 of the present of the state service of the standard.

The claim is submitted purposely through office of the service provider, in writing by mail or by means of the web portal of "the electronic government".

Confirmation of adoption of the claim is its registration (stamp, the entering number and date) in office of the service provider with indication of surname and initials of person which accepted the claim, term and the place of receipt of the answer to the made complaint.

In the claim of uslugopoluchatel its surname, name middle name, the postal address, reference number and date is entered. The claim is signed by uslugopoluchatel.

The claim of uslugopoluchatel which arrived to the service provider or the Ministry is subject to consideration within five working days from the date of its registration.

In case of disagreement with results of the rendered state service the uslugopoluchatel can address with the claim to authorized body

by assessment and control of quality of rendering the state services.

The claim of uslugopoluchatel which arrived to authorized body by assessment and control of quality of rendering the state services is considered within fifteen working days from the date of its registration.";
