About introduction of amendments to orders MZ of Ukraine of August 7, 2015 No. 494 and of December 29, 2000 No. 369

1. Bring in Item 1 of the order of the Ministry of Health of Ukraine of August 7, 2015 No. 494 "About some questions of acquisition, transportation, storage, leave, use and destruction of drugs, psychotropic substances and precursors in healthcare institutions", registered in the Ministry of Justice of Ukraine on August 26, 2015 at No. 1028/27473, the following changes:

Subitem 7 to state 1) in the following edition:

"7) form of primary accounting documentation No. 129-11/o "Leaf of purposes of medicines of drugs, psychotropic substances and precursors the patient receiving treatment in stationary or out-patient conditions and accomplishment of these appointments" and the Instruction for its filling;";

To add 2) with the new subitem 9 of the following content:

"9) form of primary accounting documentation No. 129-12/o "The magazine of registration of special prescription forms of form No. 3 (f-3) in healthcare institutions" and the Instruction for its filling.".
