For the purpose of implementation of the automated information system of civil registrations, to the population, according to articles 10 and 17 of the constitutional Law of the Kyrgyz Republic "About the Government of the Kyrgyz Republic", article 6 of the Law of the Kyrgyz Republic "About acts of civil status" the Government of the Kyrgyz Republic decides improvements of quality of the rendered services:

1. Approve:

- blank forms of statements for state registration of acts of civil status, according to appendix 1;

- blank forms of civil registrations, according to appendix 2;

- forms (the description and samples) of forms of certificates on state registration of acts of civil status, according to appendix 3;

- blank forms of the documents confirming the facts of state registration of acts of civil status according to appendix 4;

- procedure for filling of forms of civil registrations, according to appendix 5;

- procedure for filling of forms of certificates on state registration of acts of civil status according to appendix 6.

1-1. To the public registration service under the Government of the Kyrgyz Republic to perform issue of certificates on state registration of acts of civil status since December 1, 2017.
