1. For receipt of insurance indemnity under the agreement of compulsory insurance the beneficiary submits to the insurer the following documents.

1.1. The written application about payment of insurance indemnity constituted in any form, containing the details allowing to transfer money within the applied forms of clearing settlements if payment of insurance indemnity will be made via bank transfer.

1.2. Identity document of the beneficiary.

1.3. The documents confirming powers and proving the identity of person who is the legal representative or the representative of the beneficiary who is acting on the basis of the power of attorney.
