About modification and amendments in the resolution of board of Social Security Fund of the population of the Ministry of Labour and Social Protection of the Republic of Belarus of August 21, 2009 No. 12
Based on Regulations on Social Security Fund of the population of the Ministry of Labour and Social Protection of the Republic of Belarus approved by the Presidential decree of the Republic of Belarus of January 16, 2009 No. 40 "About Social Security Fund of the population of the Ministry of Labour and Social Protection" the board of Social Security Fund of the population of the Ministry of Labour and Social Protection of the Republic of Belarus DECIDES:
1. Bring in the resolution of board of Social Security Fund of the population of the Ministry of Labour and Social Protection of the Republic of Belarus of August 21, 2009 No. 12 "About approval of the document forms necessary for registration and removal from accounting of payers of compulsory insurance premiums in bodies of Social Security Fund of the population of the Ministry of Labour and Social Protection" (The national register of legal acts of the Republic of Belarus, 2009, No. 200, 10/117) the following changes and amendments:
in Item 1:
in paragraph one to replace the word "Approve" with the word "Establish";
the fourth and fifth to state paragraphs in the following edition:
"data on calculations for payments in the budget of state non-budgetary fund of social protection of the population of the Republic of Belarus according to appendix 3;
certificates of availability (absence) of debt on payments in the budget of state non-budgetary fund of social protection of the population of the Republic of Belarus according to appendix 4;";
add Item with the paragraph the sixth the following content:
"the magazine of registration of statements of payers about registration (removal from accounting) according to appendix 5.";
to state appendix 1 to this resolution in the following edition:
"Appendix 1
