On representation of the Ministry of Transport and Communications of the Republic of Belarus Item 5 of Instructions on filling of form of the state statistical observation of 6 cars (SP) (Ministry of Transport) "The questionnaire about the transportation of goods by road transport performed by the individual entrepreneur", approved by the resolution of National Statistical Committee of the Republic of Belarus of September 29, 2015 No. 134 "About approval of form of the state statistical observation of 6 cars (SP) (Ministry of Transport) "The questionnaire about the transportation of goods by road transport performed by the individual entrepreneur" and instructions for its filling" (The national legal Internet portal of the Republic of Belarus, 03.10. 2015, 7/3235) to state in the following edition:
"5. The questionnaire is constituted based on the following source accounting and other documents:
commodity-transport delivery notes;
