According to Articles 7, 20 and 23 Laws of Ukraine "About state registration of legal entities and physical persons entrepreneurs" and the subitem 26 of item 4 of the Regulations on the State committee of Ukraine concerning regulatory policy and the entrepreneurship approved by the Presidential decree of Ukraine of May 25, 2000 No. 721 "Questions of the State committee of Ukraine concerning regulatory policy and entrepreneurship" (with changes) and for the purpose of enhancement of state regulation in the field of state registration of legal entities, and also physical persons entrepreneurs, streamlining of system of the address and application of special forms of documents of the Unified State Register of Legal Entities and physical persons - entrepreneurs ORDER:

1. Approve Regulations on procedure for provision of data from the Unified State Register of Legal Entities and physical persons entrepreneurs it (is applied).

2. Approve the Procedure for execution of forms of the statement, references and certificates of reservation of the name of the legal entity from the Unified State Register of Legal Entities and physical persons entrepreneurs (are attached).

3. Approve the Form of the extract from the Unified State Register of Legal Entities and physical persons - entrepreneurs it (is applied).

4. Approve Regulations on procedure for issue of the reference from the Unified State Register of Legal Entities and physical persons entrepreneurs about reservation of the name of the legal entity it (is applied).

5. Approve reference Form from the Unified State Register of Legal Entities and physical persons - entrepreneurs it (is applied).

6. Approve reference Form from the Unified State Register of Legal Entities and physical persons - entrepreneurs about reservation of the name of the legal entity it (is applied).

7. To department of registration and licensing (Barbelyuk O. Regulations on procedure for provision of data from the Unified State Register of Legal Entities and physical persons - entrepreneurs

1. General provisions

1.1. This Provision is developed according to articles 7 and 20 of the Law of Ukraine "About state registration of legal entities and physical persons - entrepreneurs" and establishes single procedure for provision of data from the Unified State Register of Legal Entities and physical persons - entrepreneurs (further - the Unified state register) for the purpose of providing public authorities, and also participants of civil circulation with reliable information about legal entities and physical persons - entrepreneurs.

1.2. The terms below are used in the following value:

asking - public authority, the legal entity (his authorized representative), the physical person which sent request of the established sample;

criterion of search - indicator or set of indicators of the Unified state register on which search specified in information request is performed;

types of activity - types of activity on the Classification of Types of Economic Activity (CTEA) approved by the order of Gosstandart of Ukraine of October 22, 1996 No. 441.

1.3. According to request the information from the Unified state register which is created by the automated system of maintaining the Unified state register in the form of one of documents is provided to asking:

the extract from the Unified State Register of Legal Entities and physical persons - entrepreneurs (further - the statement);

references from the Unified State Register of Legal Entities and physical persons - entrepreneurs (further - the reference);

the extract from the Unified State Register of Legal Entities and physical persons - entrepreneurs (further - the statement).

1.4. The state registrars use forms of statements and the reference from the Unified state register which description and form affirm Gospredprinimatelstvom of Ukraine.

1.5. Statements and references from the Unified state register are signed by the state registrar and make sure its seal.

1.6. Accounting of requests, ensuring search of the data specified in requests, document creation is performed by the automated system of maintaining the Unified state register.

2. Acceptance of request

2.1. Acceptance of request about provision of data from the Unified state register is performed by the state registrars in executive committees of city councils of the cities of regional value or in district, district public administrations in the cities of Kiev and Sevastopol.

2.2. For receipt of the statement from the Unified state register the inquiry is sent to the state registrar by the location or the residence of asking.
