According to parts one and third Article 42-2, of part one of article 42-5 of the Law of Ukraine "About employment of the population" the Cabinet of Ministers of Ukraine decides:

1. Approve form:

statements for receipt by the employer of permission to application of work of foreigners and stateless persons according to appendix 1;

statements for prolongation of effective period of permission to application of work of foreigners and stateless persons according to appendix 2;

statements for modification of permissions to application of work of foreigners and stateless persons according to appendix 3.

2. Recognize invalid resolutions of the Cabinet of Ministers of Ukraine according to the list which is attached.

3. This resolution becomes effective from the date of its publication and is applied since September 27, 2017.
