According to article 8 of the Law of Ukraine "About licensing of certain types of economic activity", Bases of the legislation of Ukraine on health care, the Law of Ukraine "About donorship of blood and (or) its components" and the resolution of the Cabinet of Ministers of Ukraine of 14.11.2000 N1698 "About Approval of the List of Bodies of Licensing"

WE ORDER:

1. Approve:

1.1. Licensed conditions of implementation of economic activity on conversion of donor blood and its components, production of medicines from them (are applied).

1.2. Licensed conditions of implementation of economic activity on medical practice (are applied).

1.3. Licensed conditions of carrying out disinfection, disinsection, deratization works (except works on objects of veterinary control) which are applied.

2. To management of licensing of types of economic activity of Department of registration and licensing Gospredprinimatelstva in Ukraine (O. V. Efremov), to the Ministry of Health (A.P.Kartysh) in accordance with the established procedure to provide provision of the order on state registration in the Ministry of Justice of Ukraine.

3. To the deputy minister of health care of MZ of Ukraine Kartysh A. P. provide the publication of Licensed conditions in mass media.

4. Declare invalid the order of Licensed chamber of Ukraine and the Ministry of Health of Ukraine of May 11, 1999 N 44/118 "About approval of the Instruction about conditions and rules of implementation of business activity (licensed conditions) on conversion and storage of donor blood and its components, their realization and the medicines made of them and control of their observance", NLP-6/60 "About Approval of Instructions about Procedure for Issue to Subjects of Business Activity of Special Permissions (Licenses) for the Right of Implementation of Medical Practice, Conducting Medicolegal and Forensic-psychiatric Examination, Retail Sale of Medicines, Conditions and Rules of Implementation of These Activities and Control of Their Observance" registered in the Ministry of Justice of Ukraine of June 7, 1999 for N356/3649, and the order of Licensed chamber of Ukraine and the Ministry of Health of Ukraine of March 22, 1996, registered in the Ministry of Justice of Ukraine of May 6, 1996 for N 218/1243.

5. To impose control of execution of this order on the vice-chairman of the State committee of Ukraine concerning regulatory policy and entrepreneurship Tretyakov S. I., First deputy Minister of Health of Ukraine Bobyliova O. O. and deputy minister of health care of Ukraine Kartysh A. P.

These Licensed conditions are developed according to the Law of Ukraine "About donorship of blood and its components", the Law of Ukraine "About licensing of certain types of economic activity", resolutions of the Cabinet of Ministers of Ukraine: of 16.06.98 N920 "About Approval of Procedure for Conversion and Storage of Donor Blood and Its Components, Their Realization and the Medicines Made of Them", of 14.09.98 N1427 "About Approval of the Procedure for Realization for Borders of Ukraine of Components of Donor Blood and the Medicines Made of Donor Blood and Its Components, and also Export from Ukraine of Donor Blood and Its Components", of 14.11.00 N1698 "About Approval of the List of Bodies of Licensing".
