According to Article 7 and article 21 of the Law of Ukraine "About state registration of legal entities and physical persons entrepreneurs" and the Regulations on the State committee of Ukraine concerning regulatory policy and entrepreneurship approved by the Presidential decree of Ukraine of May 25, 2000 N721/2000 (with changes and amendments),
I ORDER:
1. Approve the Form of the extract from the Unified State Register of Legal Entities and physical persons - entrepreneurs which is applied.
2. Approve the Procedure for execution of the form of the extract from the Unified State Register of Legal Entities and physical litspredprinimatel which is applied.
3. (Efremov O. V.) in five-day time to provide to department of registration and licensing provision of this order on state registration in the Ministry of Justice of Ukraine.
4. This order becomes effective at the same time with entry into force of the Law of Ukraine "About state registration of legal entities and physical persons - entrepreneurs".
5. To impose control of execution of this order on the vice-chairman Tretyakov S. I.
