I ORDER:

1. N 533, registered in the Ministry of Justice of Ukraine on November 19, 1998 N 733/3173, to consider the order of State Tax Administration of Ukraine of November 10, 1998 invalid.

2. Claim form of the Certificate on payment of the single tax and the Procedure for issue of the Certificate on payment of the single tax that are applied.

3. For the purpose of economy of budgetary funds to resolve till December 31, 2000 use along with forms of the Certificate on payment of the single tax of forms of the Certificate on payment of the single tax by small business entity - physical person.

4. To provide to the chief of Head department of the taxation of physical persons Doctor S. I. provision on state registration in the Ministry of Justice of Ukraine of this order.

5. To provide to the chief of Head department on work with taxpayers Sysoyev V. O. after state registration of the order "About Approval of the Certificate on Payment of the Single Tax and the Procedure for Its Issue" the publication it in mass media.
