1. Approve application forms and messages which are attached:

1.2. The application form about replacement of the certificate on state registration of the legal entity.

1.3. The application form about replacement of the certificate on state registration of the physical person - the entrepreneur.

1.4. Sample of the message on closing of separate division.

2. And entrepreneurships (Efremov O. V.) in five-day time to provide to department of registration and licensing of the State committee of Ukraine concerning regulatory policy provision of this order on state registration in the Ministry of Justice of Ukraine.

3. This order becomes effective in ten days after its state registration in the Ministry of Justice of Ukraine.

4. To impose control of execution of this order on the vice-chairman of the State committee of Ukraine concerning regulatory policy and entrepreneurship Tretyakov S. I.
