About approval of form of the license, appendix to the license for educational activities and procedure for their registration
According to the Laws of Ukraine "About education", "About the higher education", "About licensing of certain types of economic activity"
I ORDER:
1. Determine that form of the license for educational activities is the form of the license of single sample which sample and the description are approved by the resolution of the Cabinet of Ministers of Ukraine of 20.11.2000 of N1719.
2. Approve form and the description of appendix to the license which are applied.
3. Approve the Instruction about procedure for filling of the form of the license and form of appendix to the license which is attached.
4. Determine that in connection with use of forms of licenses of single sample the procedure for their order, deliveries, storages, accounting and the reporting of expenditure is regulated by the order of the State committee of Ukraine concerning regulatory policy and entrepreneurship of 18.12.2000 of N 78, the registered Ministry of Justice of Ukraine on January 10, 2001 for N3/5194.
5. Abolish the order of the Ministry of Education of Ukraine of 01.04.96 of N100 "About Approval of the Description and Forms of Licenses for Implementation of the Educational Activities Connected with Receipt of General Average, Vocational Training, Training of Specialists of Different Skill Levels, Certificates on Accreditation and Certificates on Certification of Educational Institutions".
6. To the head of department of licensing, accreditation and nostrification Domnich V. I. provide provision of this order on state registration.
7. To impose control of execution of this order on the deputy minister Stepko M. F.

The text of the form of the license and the form of appendix to the license is printed on the computer.
Under the word "LICENSE" at distance of 1 cm the name of the body granting the license, according to the legislation fits in (the name is printed by capital letters bold-face type of the 14th size);
below the name of educational service (educational services) on which the license is granted fits in (according to the legislation). If such information on the amount cannot be placed in this column, then she fits into appendix to the license about what it is specified in the graph;
number (numbers) of the form (forms) of appendix to the license fits in on reverse side of the form of the license;
further the name of the legal entity (for the legal entity) or surname name and middle name of physical litsasubjekt of business activity fits in (for physical person);
opposite to the words "Identification Code of the Legal Entity or Identification Number of the Physical Person — the Taxpayer and Other Obligatory Payments" in figures the identification code (for legal entities) or identification number of the physical person - the taxpayer and other payments fits in respectively (for physical person);
opposite to the words "The Location of the Legal Entity or the Residence of the Physical Person — the Subject of Business Activity" the location of the legal entity or the residence of the physical person - the subject of business activity fits in;
opposite to the words "Acceptance Date and Number of the Decision on Licensing" in figures number and decision date (order) on licensing fit in;
opposite to the words "License Effective Period" in figures license effective period fits in;
below the position, surname and initials of the official who signed the license fits in;
below opposite to the words "date of issue of the license" in figures date of issue of the license fits in;
from the left edge of the form the license makes sure seal of the body which granted the license.
When filling the form of appendix to the license under the words "APPENDIX TO the LICENSE" at distance of 1 cm entry "series ___ by N ___" where series and number of the form of the license to which the form of appendix to the license is drawn up is specified is made.
In case of registration of appendix to the license for educational services in the higher education tab. 1 is used:
in the column "Direction" the code and the name of the direction of preparation according to the list approved by the resolution of the Cabinet of Ministers of Ukraine of 24.05.97 of N507 "About the List of the Directions and Specialties on Which Training of Specialists in Higher Educational Institutions on the Appropriate Educational and Qualification Levels Is Conducted" register;
in the column "Specialty" the code and the name of specialty according to the list called above register;
in the column "Licensed Amount" the corresponding licensed amount on each specialty and other educational services specified in this document registers in figures.
If in structure of higher educational institution there are structural divisions including isolated which provide educational services, then in appendix to the license all entries for divisions are made according to the procedure, established for higher educational institution.
In appendix to the license for educational services in vocational training tab. 2 is used:
in the column "Code on the State Qualifier of Ukraine" in figures the profession code according to the State qualifier of professions of recreation center 003-95, the approved order of Gosstandart of July 27, 1995 of N257 fits in;
in the column "Occupation title" the occupation title according to above the called qualifier fits in;
in the column "Licensed Amount" the corresponding licensed amount by each profession and other educational services specified in this document registers in figures.
In case of registration to use applications to the license for educational services in the field of the higher education in training of bachelors tab. 3;
in the column "Code and Name of Knowledge Domain" the code and the name of knowledge domain according to the List of the directions in which training of specialists in higher educational institutions on educational qualification level of the bachelor, by the approved resolution of the Cabinet of Ministers of Ukraine of N1719 is performed of 13.12.2006 register;
in the column "Code and Direction of Training of Bachelors" the code and the direction of training of bachelors according to the list called above are specified;
in the column "Licensed Amount" the corresponding licensed amount in each direction and form of education registers in figures".
Under the table the list of the previous decisions of the Cross-industry accreditation commission, State accreditation commission, Ministry of Education and Science of Ukraine concerning licensing is specified, to accreditation, changes of names, changes of licensed amounts and so forth which concern the educational services specified in the list;
other educational services on which the license, for example "advanced training" where the direction in which advanced training, "preparation for receipt in higher educational institutions", "training (retraining) of foreign citizens" and so forth is performed is specified is provided if necessary are specified in appendix to the license (in the table) also.
If it is more forms of appendix to the license than one, on each leaf under the words "APPENDIX TO the LICENSE" at distance of 1 cm the entry "Continuation of Appendix to the License of Series ___ N ___" where series and number of the form of the license to which the form of appendix to the license is drawn up are specified is made and number of the page of appendix is specified.

In the left bottom corner of the form six-digit number is printed. Number is printed with black paint which is shown on reverse side in red color.
