According to Articles 18, "About telecommunications" the National commission performing state regulation in the field of communication and informatization SOLVED 39 Laws of Ukraine:

1. Bring in the decision of the National commission performing state regulation in the field of communication and informatization, of April 5, 2016 No. 180 "About approval of the Procedure for provision by operators, providers of telecommunications of the reporting and information and recognition by NKRS which voided solutions of 04.11.2008 No. 1189 and of 25.12.2008 No. 1283", registered in the Ministry of Justice of Ukraine on May 18, 2016 for No. 739/28869, the following changes:

Item 1 to state 1) in the following edition:

"1. Approve such which are applied:

Procedure for provision by operators, providers of telecommunications of the reporting and information;

form of the reporting No. 1-K (quarter) "Mobile (mobile) communication and services of data transmission, including Internet access" and Instruction for its filling;

form of the reporting No. 2-K (quarter) "The fixed telephone communication" and the Instruction for its filling;

form of the reporting No. 3-K (quarter) "The fixed Internet access" and the Instruction for its filling;

form of the reporting No. 4-K (quarter) "Provision in use of channels of telecommunication, telecommunication networks and other services in the field of telecommunications" and the Instruction for its filling;

reporting No. 5-G form (annual) "Distribution of lines of the fixed telephone communication on administrative and territorial units" and the Instruction for its filling;

reporting No. 6-G form (annual) "Distribution of lines of the fixed Internet access on administrative and territorial units" and the Instruction for its filling;

form of the reporting No. 7-K (quarter) "Distribution of active identification telecommunication cards of network of mobile (mobile) communication on administrative and territorial units" and Instruction for its filling.";

2) form of the reporting No. 1-K (semi-annual) "About the main indicators of activities of the operator, provider of telecommunications" and the Instruction for its filling, form of the reporting No. 2-A (annual) "Distribution of technical means of telecommunications on administrative and territorial units" and the Instruction for its filling approved by this decision to exclude;

3) to add the decision with the following forms of the reporting and instructions for their filling which are applied:

form of the reporting No. 1-K (quarter) "Mobile (mobile) communication and services of data transmission, including Internet access" and Instruction for its filling;

form of the reporting No. 2-K (quarter) "The fixed telephone communication" and the Instruction for its filling;

2. Approve the Changes in the Procedure for provision by operators, providers of telecommunications of the reporting and information approved by the decision of the National commission performing state regulation in the field of communication and informatization, of April 5, 2016 No. 180, registered in the Ministry of Justice of Ukraine on May 18, 2016 for No. 739/28869, which are applied.

3. In accordance with the established procedure to submit to department of the economic analysis this decision on state registration to the Ministry of Justice of Ukraine.

4. This decision becomes effective from the date of its official publication.

Financial and economic indicators, and also indicators on amounts of traffic are filled with the accruing result since the beginning of year as of the end of the last day of the accounting period. Financial and economic indicators are specified excluding tax value added and collection on compulsory national pension insurance, with rounding to one sign after comma.
