According to Articles 64 - 67 Codes of civil protection of Ukraine, to the paragraph to the eighth part two of article 5 of the Law of Ukraine "About the basic principles of the state supervision (control) in the field of economic activity", the Technique of development of the unified forms of statements which are drawn up by results of holding planned (unplanned) actions of the state supervision (control) of Ukraine approved by the resolution of the Cabinet of Ministers of May 10, 2018 No. 342 (with changes), and for the purpose of realization of state policy in the field of technogenic and fire safety ORDER:

1. Approve such which are applied:

form of the statement which is drawn up by results of holding planned (unplanned) action of the state supervision (control) for observance by the subject of managing of requirements of the legislation in the field of technogenic and fire safety;

form of the statement which is drawn up concerning impossibility of implementation of action of the state supervision (control);

form of the notification on holding planned action for implementation of the state supervision (control);

certificate form on holding action of the state supervision (control);
