1. Approve such which are applied:

1) reporting No. 7 form "The report on providing persons with disability, children with disability and other separate categories of the population technical and other means of rehabilitation" (further - No. form 7);

2) the Instruction for filling of form of the reporting No. 7 "The report on providing persons with disability, children with disability and other separate categories of the population technical and other means of rehabilitation".

2. (Vishnevskaya N. V.) to provide to department of implementation of the state social programs:

1) submission of this order in accordance with the established procedure on state registration in the Ministry of Justice of Ukraine;

2) representation of statistics of Ukraine to Public service annually till March 15 of the year following for reporting, the summarized information in form No. 7 for January - December concerning Ukraine in general and on administrative and territorial units.
