According to part three of article 16 of the Law of Ukraine "About state regulation of production and the address of alcohol ethyl, cognac and fruit, alcoholic beverages and tobacco products" I ORDER:

1. Approve the following (are applied):

report form of N1-PC "Report on Production Volumes and Realization of Alcohol";

report form of N2-PC "Report on Production Volumes and Realization of Alcoholic Beverages";

report form of N3-PC "Report on Production Volumes and Realization of Tobacco Products";

Instruction for filling of report form of N1-PC "Report on Production Volumes and Realization of Alcohol";

Instruction for filling of report form of N2-PC "Report on Production Volumes and Realization of Alcoholic Beverages";

Instruction for filling of report form of N3-PC "Report on Production Volumes and Realization of Tobacco Products".

2. Filling of report forms is performed with use of codes of products and codes of areas according to appendix.

3. To administrative department () in three-day time from the date of entry into force of this order to provide to A.M. Rakhuba its replication and the direction to State Tax Administrations in the Autonomous Republic of Crimea, areas, the cities of Kiev and Sevastopol.

4. To impose control over the implementation of this order on the vice-chairman Lekar of S. I.
