I ORDER:

1. Bring in the order of the Minister of Defence of the Republic of Kazakhstan of March 14, 2017 No. 113 "About approval of standards of the state services of the Ministry of Defence of the Republic of Kazakhstan" (it is registered in the Register of state registration of regulatory legal acts for No. 15393, it is published in Reference control bank of regulatory legal acts of the Republic of Kazakhstan on August 11, 2017), the following changes:

10) the standard of the state service "Apostilization of Archive References and Copies of the Archive Documents Coming from the Central Archive of the Ministry of Defence of the Republic of Kazakhstan", according to appendix 10 to this order.";

2. To provide to department of organizational and mobilization work of the General Staff of Armed Forces of the Republic of Kazakhstan in the procedure established by the legislation of the Republic of Kazakhstan:

1) state registration of this order in the Ministry of Justice of the Republic of Kazakhstan;

2) the direction of the copy of this order in the Republican state company on the right of economic maintaining "Institute of the legislation and legal information of the Republic of Kazakhstan" the Ministries of Justice of the Republic of Kazakhstan for official publication and inclusion in Reference control bank of regulatory legal acts of the Republic of Kazakhstan in the Kazakh and Russian languages within ten calendar days from the date of state registration;

3) placement of this order on Internet resource of the Ministry of Defence of the Republic of Kazakhstan after its first official publication;

4) the direction of data in Legal department of the Ministry of Defence of the Republic of Kazakhstan on execution of the actions provided by subitems 1), 2) and 3) of Item 2 of this order within ten calendar days from the date of state registration.

3. To impose control of execution of this order on the First Deputy Minister of Defence - the chief of the General Staff of Armed Forces of the Republic of Kazakhstan.

4. Bring this order to officials in the part concerning them.

5. This order becomes effective after ten calendar days after day of its first official publication.

Standard of the state service "Issue to Recruits of Certificates of Addition to Draft Sites and Duplicates of Certificates"

Chapter 1. General provisions

1. The state service "Issue to Recruits of Certificates of Addition to Draft Sites and Duplicates of Certificates" (further - the state service).

2. The standard of the state service is developed by the Ministry of Defence of the Republic of Kazakhstan.

3. The state service appears local authorities of military management of the Ministry of Defence of the Republic of Kazakhstan (further - the service provider).

Reception of an application and issue of result of rendering the state service is performed through non-commercial joint-stock company "The Government for Citizens state corporation (further - the State corporation).

Chapter 2. Procedure for rendering the state service

4. The term of rendering the state service in case of the appeal to the State corporation (day of reception of applications and documents does not enter rendering service in time):

1) from the moment of delivery of document package:

in case of certification about addition to the draft site (further - the certificate of addition) - 5 (five) working days;

in case of issue of the duplicate of the certificate of addition - 5 (five) working days;

2) the most allowed time of expectation for delivery of document package - 15 (fifteen) minutes;

3) the most allowed time of servicing in the State corporation - 15 (fifteen) minutes.

5. Form of rendering the state service: paper.

6. Is result of rendering the state service:

1) certification about addition in case of primary address of the citizen who did not pass addition in January - March in year of execution to it seventeen years;

2) issue of the duplicate of the certificate of addition during the losing, spoil and change of surname, name, middle name;

3) the motivated answer about refusal in rendering the state service, in cases and on the bases, stipulated in Item 10 these standards of the state service.

Form of provision of result of rendering the state service: paper.

7. The state service is rendered to physical persons (further - uslugopoluchatel) free of charge.

8. Working schedule:

1) the service provider - Monday through Friday from 9:00 till 18:00 o'clock, with lunch break from 13:00 till 14:00 o'clock, except for days off and holidays, according to the labor law of the Republic of Kazakhstan;

2) the State corporation - Monday through Saturday inclusive, according to the established working schedule from 9:00 till 20:00 o'clock without interruption for lunch, except for Sundays and holidays, according to the labor law of the Republic of Kazakhstan.

Acceptance is performed in the place of permanent registration of uslugopoluchatel, without the accelerated servicing, possibly booking of electronic queue by means of the web portal of "the electronic government".

9. The list of the documents necessary for rendering the state service in case of the address of uslugopoluchatel (or his representative under the notarized power of attorney) in the State corporation:

1) in case of primary address of the uslogopoluchatel who did not pass addition in January - March in year of execution to it seventeen years:

the statement on certification for addition to the draft site and duplicates of certificates in form, according to appendix 1 to this standard of the state service;

the identity document (for identification) (data on the document, proving the identity are attached to the statement of uslugopoluchatel by the employee of the State corporation from the corresponding state information systems through lock of "the electronic government", in the presence of integration);

the statement from the book of protocols of the district (city) draft commission (it is applied by the service provider);

the certificate of birth (data on the birth are attached to the statement of uslugopoluchatel by the employee of the State corporation from the corresponding state information systems through lock of "the electronic government", in the presence of integration);

the marriage certficate (for the citizens of military age who are married) (data on scrap are attached to the statement of uslugopoluchatel by the employee of the State corporation from the corresponding state information systems through lock of "the electronic government", in the presence of integration);

the certificate of birth of children (for the citizens of military age having dependent on children) (data on the birth of children are attached to the statement of uslugopoluchatel by the employee of the State corporation from the corresponding state information systems through lock of "the electronic government", in the presence of integration);

the education document (in the presence) (data on education are attached to the statement of uslugopoluchatel by the employee of the State corporation from the corresponding state information systems through lock of "the electronic government", in the presence of integration);

photos the size 30kh40mm - 2 pieces (photos shall correspond to age of the applicant and are executed strictly in fullface on light background, person shall occupy about 70% of total area of the photo);

2) when losing the certificate of addition:

the statement on certification for addition to the draft site and duplicates of certificates in form, according to appendix 1 to this standard of the state service;

the identity document (for identification) (data on the document, proving the identity are attached to the statement of uslugopoluchatel by the employee of the State corporation from the corresponding state information systems through lock of "the electronic government", in the presence of integration);

the receipt on payment of penalty, stipulated in Article 649 Codes of the Republic of Kazakhstan "About administrative offenses" (in the presence of offense);

photos the size 30kh40mm - 2 pieces (photos shall correspond to age of the applicant and are executed strictly in fullface on light background, person shall occupy about 70% of total area of the photo);

3) in case of spoil of the certificate of addition:

the statement on certification for addition to the draft site and duplicates of certificates in form, according to appendix 1 to this standard of the state service;

the identity document (for identification) (data on the document, proving the identity are attached to the statement of uslugopoluchatel by the employee of the State corporation from the corresponding state information systems through lock of "the electronic government", in the presence of integration);

the original of the available certificate of addition;

the receipt on payment of penalty, stipulated in Article 649 Codes of the Republic of Kazakhstan "About administrative offenses" (in the presence of offense);

photos the size 30kh40mm - 2 pieces (photos shall correspond to age of the applicant and are executed strictly in fullface on light background, person shall occupy about 70% of total area of the photo);

4) in case of change of surname, name, middle name:

the statement on certification for addition to the draft site and duplicates of certificates in form, according to appendix 1 to this standard of the state service;

the identity document (for identification) (data on the document, proving the identity are attached to the statement of uslugopoluchatel by the employee of the State corporation from the corresponding state information systems through lock of "the electronic government", in the presence of integration);

the original of the available certificate of addition;

photos the size 30kh40mm - 2 pieces (photos shall correspond to age of the applicant and are executed strictly in fullface on light background, person shall occupy about 70% of total area of the photo).

Uslugopoluchatel agrees to use of the data which are the secret protected by the law, containing in information systems when rendering the state services if other is not provided by the laws of the Republic of Kazakhstan.

In case of documents acceptance through the State corporation the receipt on acceptance of the relevant documents is issued to uslugopoluchatel.

In the State corporation issue of ready documents is performed based on the receipt on acceptance of the relevant documents, in case of presentation of the identity document (or his representative under the notarized power of attorney).

The state corporation provides storage of result within one month then transfers him to the service provider for further storage. In case of the address of uslugopoluchatel on the expiration of one month, at the request of the State corporation the service provider within one working day sends ready documents to the State corporation for issue to uslugopoluchatel.

10. The service provider refuses provision of the state service in the following bases:

1) establishment of unauthenticity of the documents submitted by uslugopoluchatel for receipt of the state service, and (or) these (data) containing in them;

2) discrepancy of uslugopoluchatel and (or) the provided materials, objects, supplied and the information necessary for rendering the state service, to the requirements established by regulatory legal acts of the Republic of Kazakhstan;

3) lack of entry in the book of protocols of the draft commission, for category of the citizens provided by the subitem 1 of Item 9 of this standard.

In case of representation by uslugopoluchatel of incomplete document package according to the list, stipulated in Item 9 these standards of the state service, the employee of the State corporation refuses reception of an application and issues the receipt on refusal in documents acceptance in form, according to appendix 2 to this standard of the state service.

Chapter 3. Procedure for appeal of decisions, actions (failure to act) of the service provider and (or) his officials, the State corporation and (or) their workers concerning rendering the state services

11. Appeal of decisions, actions (failure to act) of the service provider and (or) his officials, the State corporation and (or) their workers concerning rendering the state services:

1) the claim is submitted addressed to the head of local body of military management to the address and phones specified on Internet resource of the Ministry of Defence of the Republic of Kazakhstan to www.mod.gov.kz.

The claim is submitted in writing by mail or purposely through office of the service provider, according to the working schedule.

Confirmation of adoption of the claim is its registration (stamp, the entering number and date) in office of the service provider, with indication of surname and initials of person which accepted the claim, term and the place of receipt of the answer to the made complaint;

2) the claim to actions (failure to act) of the employee of the State corporation goes uslugopoluchatel by the address to the head of the State corporation to the addresses and phones specified in Item 15 of this standard of the state service.

Confirmation of adoption of the claim in the State corporation which arrived as if on purpose, and mail, is its registration (stamp, the entering number and registration date are put down on the second copy of the claim or in the cover letter to the claim).

The claim of uslugopoluchatel which arrived to the service provider or the State corporation is subject to consideration within five working days from the date of its registration. The motivated answer about results of consideration of the claim goes to uslugopoluchatel by mail, or is issued purposely in office of the service provider.

In case of disagreement with results of the rendered state service, the uslugopoluchatel can address with the claim to authorized body by assessment and control of quality of rendering the state services.

The claim of uslugopoluchatel which arrived to authorized body by assessment and control of quality of rendering the state services is subject to consideration within fifteen working days from the date of its registration.

12. In case of disagreement with results of the rendered state service, the uslugopoluchatel has the right to take a legal action in the procedure established by the legislation of the Republic of Kazakhstan.

Chapter 4. Other requirements taking into account features of rendering the state service through the State corporation

13. Uslugopoluchatelyam, having violation of health with permanent disorder of functions of organism limiting its life activity, in case of need documents acceptance for rendering the state service, is made by the employee of the State corporation with departure at the place of residence by means of the address through Single contact center 1414, 8 800 080 7777.

14. Uslugopoluchatel has possibility of receipt of information on the status of rendering the state service in the mode of remote access by means of service desk of the service provider, Single contact center 1414, 8 800 080 7777.

15. Addresses of places of rendering the state service are placed on Internet resource of the Ministry of Defence of the Republic of Kazakhstan: www.mod.gov.kz, and also on Internet resource of the State corporation: www.gov4c.kz.

16. Information on procedure for rendering the state service can be obtained by phone Single contact center 1414, 8 800 080 7777.
