About determination of document forms on registration of topology of integral chip

1. Establish forms:

statements for registration of topology of integral chip according to appendix 1;

statements for modification of specifying of authors of topology of integral chip in connection with change of list of authors according to appendix 2;

statements for modification of specifying of authors of topology of integral chip in connection with change of surname, own name, middle name (if that is available) the author (authors) and (or) the address of its residence (the place of stay) according to appendix 3;

statements for modification of specifying of the applicant upon transition of the right to registration of topology of integral chip as a result of legal succession according to appendix 4;

statements for modification of specifying of the applicant by transfer of the right to registration of topology of integral chip under the agreement according to appendix 5;
