General provisions

1. This Instruction "About procedure for registration, issue and accounting of the passport of the citizen of the Kyrgyz Republic of sample of 2004" (further in the text - the Instruction) is developed according to the Provision "About the Passport of the Citizen of the Kyrgyz Republic" approved by the order of the Government of the Kyrgyz Republic of July 27, 2004 No. 557.

2. The instruction determines procedure for acceptance, registration and verification of documents for production of the passport of the citizen of the Kyrgyz Republic of sample of 2004 (further - the passport), and also issues of the made passport to citizens of the Kyrgyz Republic in the territory of the Kyrgyz Republic.

3. The Ministry of Internal Affairs of the Kyrgyz Republic performs acceptance and document creation from citizens of the Kyrgyz Republic for production of passports, and also issues to citizens of the Kyrgyz Republic the made passports.

4. The state agency of information resources and technologies under the Government of the Kyrgyz Republic (further - Gosinformresursov) provides production of passports and creates the information database about the issued passports.

5. Verification of the documents submitted for production of the passport is performed by law-enforcement bodies of the Kyrgyz Republic.

6. The ethnic Kyrgyz returning to the historical homeland from the Republic of Tajikistan receive the passport according to Items 49-51 of this Instruction.

9. For obtaining, exchange of the passport the citizen of the Kyrgyz Republic represents to passport division of law-enforcement bodies of the Kyrgyz Republic (further - territorial passport subdivision) at the place of residence the following documents:

9.1. for the citizens who reached 16-year age:

- the statement on the receipt (exchange) of the passport of the established form (further - the application form) which is filled in and signed by the applicant (Appendix No. 1);

- the certificate of birth or the statement from the birth record;

- passports of parents (legal representatives) or one of parents (in case of lack of the second) for determination of nationality of the applicant, or the reference of Management of passport and visa work of the Ministry of Internal Affairs (further UPVR Ministry of Internal Affairs), departments of passport and visa work - groups of passport and visa work of areas and Osh (further OPVR-GPVR) about belonging to nationality of the Kyrgyz Republic;

- the assigned certificate (or the document replacing it) for persons liable for call-up;

- the certificate of social protection issued by Social fund;

- 2 (two) color photos on white background of 4x6 cm in size, on glossy paper with the accurate image of person without headdress and points;

If the applicant has any natural or other special signs which are displayed in the photo (scar, birthmarks, etc.), the citizen shall specify about it with own hand in the statement of any form;

- receipt on payment for the application blank;

- receipt on payment for production of the passport;

- receipt on payment of the state fee. 9.2. for the citizens for the first time receiving the passport:

- the application form which is filled in and signed by the applicant;

- the passport of sample of 1994, either the passport of sample of 1974, or the identity certificate of the citizen of the Kyrgyz Republic, or the certificate of release from places of detention, or the certificate of birth or the statement from the birth record, either the reference No. form 9, or the certificate of UPVR Ministry of Internal Affairs, OPVR-GPVR of areas and Osh of belonging to nationality of the Kyrgyz Republic;

- the military ID, or the assigned certificate (or the document replacing it) for persons liable for call-up;

- the certificate of social protection issued by Social fund;

- the certificate of UPVR Ministry of Internal Affairs of the Kyrgyz Republic of acceptance or recovery of the applicant in nationality of the Kyrgyz Republic (in Item 20 of the application blank number and date of the corresponding Presidential decree of the Kyrgyz Republic is specified);

- certificate of birth (in the presence);
