ORDER OF THE MINISTER OF DEFENCE OF THE REPUBLIC OF KAZAKHSTAN

About approval of Rules of medical support of flights of the state aircraft of the Republic of Kazakhstan

(as amended on 13-05-2024)

Based on the subitem 25) of article 15 of the Law of the Republic of Kazakhstan of July 15, 2010 "About use of airspace of the Republic of Kazakhstan and activities of aircraft" I ORDER:

1. Approve the enclosed Rules of medical support of flights of the state aircraft of the Republic of Kazakhstan.

2. To provide to management of the commander-in-chief Silami of air defense of Armed Forces of the Republic of Kazakhstan in the procedure established by the legislation of the Republic of Kazakhstan:

1) state registration of this order in the Ministry of Justice of the Republic of Kazakhstan;

2) the direction in the Republican state company on the right of economic maintaining "Institute of the legislation and legal information of the Republic of Kazakhstan" the Ministries of Justice of the Republic of Kazakhstan for official publication and inclusion in Reference control bank of regulatory legal acts of the Republic of Kazakhstan in the Kazakh and Russian languages within ten calendar days from the date of state registration;

3) placement of this order on Internet resource of the Ministry of Defence of the Republic of Kazakhstan after its first official publication;

4) the direction of data in Legal department of the Ministry of Defence of the Republic of Kazakhstan on execution of the actions provided by subitems 1), 2) and 3) of this Item within ten calendar days from the date of state registration.

3. To impose control of execution of this order on heads of governing bodies of the state aircraft of the Republic of Kazakhstan.

4. Bring this order to officials in the part concerning them.

5. This order becomes effective after ten calendar days after day of its first official publication.

Rules of medical support of flights of the state aircraft of the Republic of Kazakhstan

Chapter 1. General provisions

1. These rules of medical support of flights of the state aircraft of the Republic of Kazakhstan (further - Rules) determine procedure for medical support of flights of the state aircraft of the Republic of Kazakhstan.

2. Medical support of flights is performed for the purpose of preserving health and efficiency of aviation personnel for the benefit of effective and safe implementation of flight tasks in case of production of flights.

2-1. In these rules the following concepts are used:

1) aviation personnel – the persons having special and (or) professional training, performing activities for accomplishment and ensuring flights of aircrafts, airborne transportations and aviation works, maintenance of aircrafts, the organization and servicing of air traffic, air traffic control;

2) air unit – the republican public institution of the state aircraft of the Republic of Kazakhstan performing the organization and accomplishment of flights of aircrafts of the state aircraft, their operation and storage;

3) military-medical (medical) divisions – structural divisions of the central executive bodies and other central state bodies and their territorial subdivisions, and also military-medical (medical) organizations (organizations), other divisions performing military-medical (medical) providing;

4) military-medical (medical) providing – the complex of actions including the organization and rendering the military-medical (medical) help, performing medical examinations, ensuring sanitary and epidemiologic wellbeing, supply with medicines and medical products, conducting examination in the field of health care and also scientific and methodical developments and training in military medicine in troops, divisions and departments of special state and law enforcement agencies for the purpose of recovery of fighting capacity and working ability of staff;

5) military-medical service – set of military-medical (medical) divisions in which the laws of the Republic of Kazakhstan provide the military service or special type of public service intended for military-medical (medical) ensuring activities of these bodies;

6) medical examination – establishment or confirmation of availability or absence at aviation personnel of disease, determination of the state of health, and also temporary disability, professional suitability to work.

2-2. Medical support of flights is performed by military-medical service or military-medical (medical) division of military unit or organization of the state aircraft of the Republic of Kazakhstan (further – health service of air unit).

2-3. Medical support of flights the chief of military-medical service of military unit (organization) or the health worker will organize, it replacing (further – the chief of health service of air unit).

Chapter 2. Procedure for medical observation of the state of health of aviation personnel

3. Medical observation of the state of health of aviation personnel is performed for the purpose of:

1) early identification of changes in the state of health and working capacity, establishment of communication of these changes with professional activity and holding necessary preventive, medical and rehabilitation actions;

2) identifications and studying of the factors of professional activity making adverse effect on functional condition, the state of health, working capacity and taking measures to their elimination;

3) studying of psychophysiological features;

4) studying and preventions of aviation incidents and incidents;

5) studying of reactions of organism to flights (portability of flights);

6) studying of labor organization, rest and food.

4. Medical observation of the state of health of aviation personnel includes:

1) daily observation of the state of health in case of accomplishment of professional activity;

2) periodic medical examinations;

3) medical examinations during flights (prereplaceable, selective).

5. The aviation personnel decreasing and the newcomer in air units, and also on arrival from the main labor leave and business trips (24 calendar days and more) or treatments, have extraordinary (unplanned) medical examination.

6. Aviation personnel (pilots and navigators) who are retrained and mastering fighting application on the new aircraft equipment and aviation personnel, the newcomer in air unit within the first 3 months have medical examination monthly.

7. The pilots, navigators, onboard conductors, cadets (listeners) of flight educational institutions and officials performing functions in flight as a part of crew of the aircraft (further – aircrew), persons performing management and management of flights, operators of air traffic control, operators of the pilotless aircraft (further – BVS) and parachutists with the revealed diseases or injuries interfering accomplishment of professional activity are discharged of flights, management of flights, air traffic control, managements of BVS or parachute jumps and goes to military-medical (medical) divisions of military unit or organization (further – medical divisions of air unit) to physical examination or treatment.

The health service of air unit in case of the admission of aircrew and parachutists to professional activity is guided by approximate terms of the admission to professional activity after recovery after the postponed diseases and injuries according to appendix 1 to these rules.

8. To the aviation personnel having variations in the state of health with the preventive or medical and improving purpose the medical physical culture is appointed. The aircrew and parachutists needing for health reasons restriction of physical activity and who entered the list of group of medical physical culture are not allowed to flights and parachute jumps.

9. To aircrew periodic medical examinations are performed in medical division of air unit once in 3 months according to the plan of dynamic observation of aviation personnel of the state aircraft, approved as the commander of military unit or the chief of organization (further – the commander of air unit) in form, according to appendix 1-1 to these rules.

10. The aircrew, persons performing management and management of flights, to operators of air traffic control, operators of BVS and parachutists is 6 months old not later, after the next medical examination in medical division of air unit medical examination with participation of the therapist and neuropathologist is performed. On medical indications profile specialists are attracted.

10-1. The aviation land specialists in servicing of the aircraft equipment performing activities for land ensuring flights of aircrafts, aviation works, maintenance of aircrafts, operation of land technical means of air traffic control, navigation of landing and bonds have routine medical examination once a year.

11. Results of medical examinations are entered in the medical book of aviation personnel of the state aircraft of the Republic of Kazakhstan (aircrew, persons performing management and management of flights, operators of air traffic control, operators of pilotless aircrafts, parachutists) in form, according to appendix 1-2 to these rules, in chronological procedure, at the same time the following data are reflected:

1) date of survey;

2) claims to the state of health;

3) the postponed diseases (injuries) during the period between medical examinations and their outcome;

4) studying of reactions of organism to flights (portability of flights);

5) influence of the available disease on working capacity and quality of accomplishment of flight tasks;

6) results of anthropometrical measurements and researches of functional capability of cardiovascular system;

7) shortly objective data of physical examination. At persons having variations in the state of health the objective data according to nature of disease and change which happened for the period between surveys are entered;

8) result of functional load test;

9) conclusion about the state of health and drawing of the diagnosis;

10) the decision on the admission to professional activity.

11-1. Requests of aviation personnel for medical care, results of physical examination, the conducted treatment, medical examinations on recovery are entered in the insert to the medical book of aviation personnel of the state aircraft of the Republic of Kazakhstan (aircrew, persons performing management and management of flights, operators of air traffic control, operators of pilotless aircrafts, parachutists) in form, according to appendix 1-3 to these rules.

12. Prereplaceable medical examination (further - pre-flight medical examination) includes:

1) verification of presence of the certificate on passing of the medical and flight commission;

2) individual poll about health, observance of the mode of work, rest and food, availability of claims to the state of health;

3) identification of variations in emotional condition and exhaustion signs (on appearance, behavior, communication form);

4) measurement of body temperature;

5) survey of open integuments and visible mucous, pharynx research, check of nasal breath;

6) research of arterial pulse on beam artery (within 30 seconds). The aviation personnel in case of pulse rate to 50 or over 90 beats per minute are discharged of flights, management of flights or parachute jumps;

7) measurement of arterial pressure. The aviation personnel are allowed to flight, management of flights or parachute jumps in case of the following indicators of arterial pressure: systolic - no more than 140 and at least 100 millimeters of mercury, diastolic - no more than 90 and at least 60 millimeters of mercury.

On medical indications the amount of pre-flight medical examination extends due to application of additional techniques, including test for indication of alcohol, narcotic and psychotropic substances.

13. Spot-check medical examination is performed to aviation personnel between flights and after flights in volume of pre-flight medical examination (further - interflight and postflight medical examination). According to indications interflight (postflight) medical examination extends at the expense of additional techniques or is reduced to individual poll and external survey.

14. Interflight (postflight) medical examination is performed to pilots and navigators:

1) interflight (postflight) medical examination is performed to pilots and navigators:

1) retrained and mastering fighting application on the new aircraft equipment – during 10 flight changes;

2) the newcomer in air unit – during 5 flight changes;

3) started flights after stationary or out-patient treatment – during 2 flight changes;

4) the first year of service in air unit after release – at least once a month during 1 year of flight work in air unit;

5) after breaks in flight work more than 4 months – during 5 flight changes;

6) interesting the doctor (health worker) from the point of view of individual portability of flights (flights of night vision wearing spectacles, with overloads more than 7 units, with refill of the airplane in air, at extremely small heights with rounding of land relief, in the conditions of landing minimum, after leaves or business trips).

15. The aviation personnel coming on duty have medical examination in volume of pre-flight medical examination. At the same time the mode of work, rest and food is similar to the mode before flights.

16. In case of reduction of air unit in the highest degrees of combat readiness, need of urgent departure, pre-flight medical examination it is reduced to individual poll and external survey. Poll and external inspection is performed out of system.

17. Results of medical examinations of aviation personnel (preflight, interflight or postflight, before zastupleniye on alert, in case of accomplishment of parachute jumps or training ejections, before special researches and trainings) are fixed in the book of accounting of results of medical examination in form according to appendix 2 to these rules.

18. By results of medical examination the following decisions are passed:

1) about the admission to professional activity;

2) about temporary discharge;

3) about holding preventive, medical and rehabilitation actions;

4) about need of provision of rest;

5) about the direction on consultation, physical examination or treatment;

6) about extraordinary medical examination.

Chapter 3. Procedure for medical support during preparation and carrying out flights

19. Medical support during preparation and carrying out flights is performed for the purpose of timely identification and discharge from flights, management of flights, parachute jumps or servicing of the aircraft equipment of persons with initial forms sharp or exacerbation of chronic diseases, with decreased work capacity.

20. Medical support during preparation for flights includes:

1) studying of the planned table of flights on compliance of the planned flight loading for health reasons and physical endurance of the aviation personnel planned for flights;

2) planning of medical support of flights in accordance with the terms and nature of the forthcoming flights;

3) informing aviation personnel on psychophysiological features of the forthcoming flights in form of work (conversations), teaching aviation medicine and survival in the conditions of autonomous existence, special researches and trainings in relation to nature of the planned flights;

4) choice of protective equipment and its adjustment, check of operation, sanitary and hygienic condition and storage conditions of protective equipment;

5) check of completeness and condition of property of medical post of airfield, medical property of rescue paraborne group and land search and rescue team, onboard medical first-aid kits and medicamentous group of emergency inventory;

6) check of compliance of food to nature of the forthcoming flights;

7) check of full value of diet and observance of the mode of preflight food;

8) before filling of the aircraft with oxygen, the oxygen check (supporting documents its quality) given for gas station of life support system on the aircraft according to appendix 1-4 to these rules, and also hygienic condition of means of gas station (charging) of oxygen which influence providing accident-free aircrafts of operation.

21. For carrying out flights the doctor on duty (health worker) is appointed to flights and the doctor (health worker) in structure of rescue paraborne group, the medical post of airfield is exposed. The doctor on duty (health worker) on flights at the same time heads medical post of airfield and participates in search and rescue works as a part of land search and rescue team.

22. The medical post of airfield intends for rendering emergency medical service in airfield, evacuation of patients and victims in medical divisions of air unit or the medical organizations (irrespective of patterns of ownership).

The medical post of airfield is equipped with the ambulance and the corresponding medical property. The ambulance of medical post of airfield is not allowed to be used not directly.

23. When basing air unit in several airfields the complex of actions of medical support of flights will be organized by the chief of health service of air unit, and performed by the health worker of air unit having the admission to medical support of flights.

24. During preparation and carrying out flights the doctor on duty (health worker):

1) is performed by pre-flight, interflight and postflight medical examination;

2) is checked by sanitary and hygienic condition of places of meal and rest of aviation personnel in airfield.

25. Pre-flight medical examination of aviation personnel is performed in individual procedure, without presence of strangers in the room of pre-flight medical examination.

26. The aviation personnel (technical structure) providing preparation of the aircraft equipment for flights are exposed to individual poll and external survey (according to indications medical examination in volume of pre-flight medical examination is performed).

27. The aviation personnel are not allowed (are discharged) to accomplishment of flights, management of flights, parachute jumps or ensuring flights:

1) without performing pre-flight medical examination (poll);

2) in case of presentation of claims to the state of health, decrease in working capacity;

3) with violations of the preflight mode of work, rest and food;

4) with the revealed variations in the state of health.

28. The decision of the official performing pre-flight medical examination on discharge from flights, management of flights, parachute jumps or ensuring flights is final and it is obligatory for accomplishment.
