1. Bring in the order of the Government of the Republic of Kazakhstan of June 8, 2011 No. 645 "About approval of Rules of accreditation of subjects of scientific and (or) scientific and technical activities" (SAPP of the Republic of Kazakhstan, 2011, No. 41, the Art. 531) the following changes:

in Rules of accreditation of the subjects of scientific and (or) scientific and technical activities approved by the specified resolution:

9, of 10, 11 and 12 to state Items in the following edition:

"9. For receipt of accreditation legal entities submit to authorized body the following documents:

1) the statement in form according to appendix 2 to these rules;

2) rating leaf;

3) notarized copy of articles of organization; the copy of the certificate on state registration of the organization as the legal entity or the certificate of state registration (re-registration) of the legal entity;

4) copies of the documents confirming the data specified in rating leaf over the last 5 years.

If the period of activities of the subject does not exceed 5 years, copies of supporting documents are represented for the entire period of its activities.

10. For receipt of accreditation physical persons submit to authorized body the following documents:

1) the statement in form according to appendix 3 to these rules;
