The field is filled automatically after holding electronic procedure of confirmation of identification of person contractor which directly constituted VDP on behalf of person, data on whom are entered in the field 3/18 or 3/20 (further - the customs applicant/representative).
The name, surname, series (in the presence) and passport number, contact telephone number (in the presence) of person contractor are entered.

The issued documents, certificates and permissions, additional references

Data on the documents submitted according to  subitems "b", "g" of Item of 1 part one of Article 335 of the Customs code of Ukraine are specified (further - I WEAVE).
If when moving the international express departure transportation is carried out according to two or more transport (transportation) documents according to the transport agreements Master and house level, data about such the Master and house documents are specified.
In the presence data relatively are in addition specified: receipts of authorization, availability of permissions, books of MDP (TIR carnet), the preliminary or periodic customs declaration, provisional solutions concerning classification of goods, determination of the country of goods' origin.
Data concerning each document are specified from new line: document code according to the qualifier of documents, certificates, permissions and the additional information which affirms the Ministry of Finance of Ukraine (further - the Qualifier of documents), document number.
If the document is absent in appropriate section of the Qualifier of documents, in VDP the code of not classified document of appropriate section is specified.
If to the document there corresponds the code of not classified document, before document number its name which shall allow to identify the document is specified.

LRN (local help number)

The help number VDP assigned by the customs applicant/representative which is created according to the following scheme is specified: "DDDDDDDDDDDDDDDDDNNNNN", where:
"ДДДДДДДДДДДДДДДДД" - the accounting number specified in the field 3/18 or 3/20 VDP if such number at carrier is absent, is specified "99999999999999999";
"HHHHH" - the direct sequence number of VDP which is got in the accruing procedure by means of own system of accounting of representation of general declarations of arrival to customs authorities entered by the customs applicant/representative and is unique (cannot repeat) by date of forming of help number VDP.

If VDP is given by carrier, the accounting number of person assigned by customs authority got according to part one of article 455 WEAVE by the carrier moving goods is specified.
If the carrier does not stay on the registry according to part one of article 455 WEAVE, the field is not filled, the information about carrier which VDP moves is specified in the field 3/31 VDP.
The field is not filled if data in the field of 3/20 VDP are entered.

If instead of GPD the customs declaration which contains necessary data for carrying out risk analysis for the purpose of assessment of safety and reliability moves, the accounting number assigned by customs authority of the country where the customs declaration, person which submitted such customs declaration was adopted is specified.
