1. Approve the application form of public organization of persons with disability about provision to the company, the organization which are founded by public organizations of persons with disability, permissions to right to use by tax benefits which is applied.

2. Make change to preamble of the order of the Ministry of social policy of Ukraine of September 5, 2013 No. 545 "Some questions of provision of government assistance to the companies and the organizations which are founded by public organizations of disabled people", registered in the Ministry of Justice of Ukraine on September 19, 2013 for No. 1620/24152, having excluded words and figures "paragraph two of Item 3 of the Procedure for provision of the permission to right to use by tax benefits for the companies and the organizations of public organizations of disabled people approved by the resolution of the Cabinet of Ministers of Ukraine of August 8, 2007 No. 1010 (in edition of the resolution of the Cabinet of Ministers of Ukraine of July 17, 2013 No. 518)".
