1. Make changes in:

form of the tax declaration of the payer of the single tax - physical person - the entrepreneur approved by the order of the Ministry of Finance of Ukraine of June 19, 2015 No. 578, No. 799/27244 registered in the Ministry of Justice of Ukraine on July 07, 2015 (with changes), having been reworded as follows it which is applied;

the form of the tax declaration of the payer of the single tax of the fourth group approved by the order of the Ministry of Finance of Ukraine of June 19, 2015 No. 578, No. 799/27244 registered in the Ministry of Justice of Ukraine on July 07, 2015 (with changes), having been reworded as follows it which is applied.

2. Tax declarations of the payer of the single tax - physical person - the entrepreneur and the payer of the single tax of the fourth group move in the forms approved by this order for the tax accounting periods of 2021.
