form of the tax declaration of the payer of the single tax of the third group (legal entity);

form of the tax declaration of the payer of the single tax of the fourth group.

2. Recognize to invalid:

the order of the Ministry of Finance of Ukraine of December 21, 2011 No. 1688 "About approval of forms of tax declarations of the payer of the single tax", registered in the Ministry of Justice of Ukraine on December 27, 2011 for No. 1533/20271;

the order of the Ministry of Finance of Ukraine of November 7, 2012 No. 1159 "About modification of forms of the tax declarations of the payer of the single tax approved by the order of the Ministry of Finance of Ukraine of December 21, 2011 No. 1688", registered in the Ministry of Justice of Ukraine on November 28, 2012 for No. 1998/22310;
