Based on paragraph one of Item 2 and Item 3 of the Regulations on procedure for creation of calculation of amount of remuneration of the organizer for the commission agreement (order) approved by the resolution of Council of Ministers of the Republic of Belarus of December 15, 2022 No. 869, of subitem 9.1 of Item 9 of the Regulations on the Ministry of Health of the Republic of Belarus approved by the resolution of Council of Ministers of the Republic of Belarus of October 28, 2011 No. 1446, the Ministry of Health of the Republic of Belarus DECIDES:
1. Determine form of calculation of amount of remuneration of the organizer for the commission agreement (order) according to appendix.
2. Approve the Instruction about procedure for filling of form of calculation of amount of remuneration of the organizer for the commission agreement (order) it (is applied).
3. Recognize invalid:
the resolution of the Ministry of Health of the Republic of Belarus of April 4, 2019 No. 25 "About determination of form of calculation of amount of remuneration of the organizer for the commission agreement and procedure for its filling";
the resolution of the Ministry of Health of the Republic of Belarus of April 19, 2019 No. 36 "About change of the resolution of the Ministry of Health of the Republic of Belarus of April 4, 2019 No. 25";
the resolution of the Ministry of Health of the Republic of Belarus of September 12, 2019 No. 94 "About change of the resolution of the Ministry of Health of the Republic of Belarus of April 4, 2019 No. 25".
