About modification of some decisions of the Government of the Kyrgyz Republic in the field of civil registration

For the purpose of realization of part 3 of article 41 of the Law of the Kyrgyz Republic "About acts of civil status", according to articles 10 and 17 of the constitutional Law of the Kyrgyz Republic "About the Government of the Kyrgyz Republic" the Cabinet of Ministers of the Kyrgyz Republic decides:

1. Bring in the order of the Government of the Kyrgyz Republic "About the Unified state register of the population of the Kyrgyz Republic and the automated information system of civil registrations of the Kyrgyz Republic" of October 21, 2013 No. 573 the following changes:

in the name, preamble and in paragraph three of Item 2 of the text in state language of the word shall be replaced with words "organdaryndaga zhazua" "жазуунун";

in Regulations on the Unified state register of the population of the Kyrgyz Republic approved by the above-stated resolution:

- in the paragraph the second the subitem 2 of Item 9 of the word "paternity proof, adoption (adoption);" to exclude;

in the Regulations on the automated information system of civil registrations of the Kyrgyz Republic approved by the above-stated resolution:

- in the name, in the paragraph the second Item 2 of the text in state language of the word shall be replaced with words "organdaryndaga zhazua" "жазуунун";

- to recognize paragraphs of the fifth and sixth of Item 8 invalid.

2. Bring in the order of the Government of the Kyrgyz Republic "About approval of blank forms of statements for state registration of acts of civil status, blank forms of civil registrations and procedure for their filling, forms (the description and samples) of forms of certificates on state registration of acts of civil status and procedure for their filling and blank forms of the documents confirming the facts of state registration of acts of civil status" of December 11, 2018 No. 579 the following changes:

1) in appendix 1 to the above-stated resolution:

- blank forms of statements for state registration of acts of civil status No. 2, of 3, 3a, 3b the text in state language to state in edition according to appendix 1 to this resolution;

- add with forms of the form of the document confirming the fact of state registration of the act of civil status No. 2a, 3v in edition according to appendix 2 to this resolution;

- blank forms of statements for state registration of acts of civil status No. 4, 5, 5a, 5b in edition according to appendix 3 to this resolution;

2) in appendix 2 to the above-stated resolution:

- blank forms of civil registrations No. 2, 2a, 3a to state 3, in edition according to appendix 4 to this resolution;

- blank forms of civil registrations No. 4, 4a, 5, 5a to recognize invalid;

3) in appendix 3 to the above-stated resolution:

- to state forms (the description and samples) of forms of certificates on state registration of the conclusion and annulment of marriage in edition according to appendix 5 to this resolution;

- to recognize forms (the description and samples) of forms of certificates on state registration of adoption (adoption) and paternity proof invalid;

4) in appendix 4 to the above-stated resolution:

- to state blank forms of the documents confirming the facts of state registration of acts of civil status No. 2, 3 in edition according to appendix 6 to this resolution;

- to recognize blank forms of the documents confirming the facts of state registration of acts of civil status No. 4, 5 invalid;

- to state form of the form of the document confirming the fact of state registration of the act of civil status No. 11 in edition according to appendix 7 to this resolution;

- add with form of the form of the document confirming the fact of state registration of the act of civil status No. 11a in edition according to appendix 8 to this resolution;

5) in appendix 5 to the above-stated resolution:

- procedure for filling of forms of civil registrations in No. forms 2, 2a, 3a to state 3, in edition according to appendix 9 to this resolution;

- procedure for filling of forms of civil registrations in No. forms 4, 4a, 5a to declare 5, invalid;

6) in appendix 6 to the above-stated resolution:

- to state procedure for filling of forms of certificates on the conclusion and annulment of marriage in edition according to appendix 10 to this resolution;

- and about paternity proof to declare procedure for filling of forms of certificates on adoption (adoption) invalid.

3. To the ministry of digital development of the Kyrgyz Republic to take the adequate measures following from this resolution.

Determine that earlier made registration certificates of acts of civil status approved by the order of the Government of the Kyrgyz Republic "About approval of blank forms of statements for state registration of acts of civil status, blank forms of civil registrations and procedure for their filling, forms (the description and samples) of forms of certificates on state registration of acts of civil status and procedure for their filling and blank forms of the documents confirming the facts of state registration of acts of civil status" of December 11, 2018 No. 579, are recognized valid and are subject to issue to complete expenditure of remaining balance of blanks products.

5. To impose control of execution of this resolution on department of digital development of Administration of the President of the Kyrgyz Republic.

6. This resolution becomes effective after seven days from the date of official publication.
