I ORDER:

1. Bring in the order of the Minister of national economy of the Republic of Kazakhstan of January 6, 2015 No. 4 "About approval of forms of notifications and Regulations of Admission of notifications by state bodies, and also about determination of the state bodies performing acceptance of notifications" (it is registered in the Register of state registration of regulatory legal acts for No. 10194) the following changes and amendments:

to state preamble of the order in the following edition:

"According to the subitem 4) of article 11 of the Law of the Republic of Kazakhstan "About permissions and notifications" I ORDER:";

state Item 1 in the following edition:

"1. Approve enclosed:

1) Regulations of Admission of notifications state bodies according to appendix 1 to this order;

2) form of the notification on the beginning or the termination of implementation of activities or certain actions according to appendix 2 to this order;

3) form of the notification on the beginning of production of installation and construction works according to appendix 3 to this order;

4) form of the notification on the beginning or the termination of activities of self-regulatory organization according to appendix 3-1 to this order;

5) form of the notification on the beginning or the termination of activities for artificial animal husbandry which types are included in appendices I and II of the Convention on international trade in the types of wild fauna and flora which are under the threat of disappearance according to appendix 3-2 to this order;

6) form of the notification on the beginning or the termination of activities for energy examination according to appendix 3-3 to this order;

7) form of the notification on the beginning of activities as the individual entrepreneur according to appendix 3-5 to this order;

8) form of the notification on the beginning or the termination of activities as the taxpayer performing separate types of activity on gasoline production (except aviation), diesel fuel, wholesale and (or) retail sale of gasoline (except aviation), diesel fuel according to appendix 3-6 to this order;

9) form the notification on the beginning or the termination of activities as the taxpayer performing separate types of activity on wholesale of tobacco products according to appendix 3-7 to this order;

10) form of the notification on the beginning or the termination of activities as the taxpayer performing separate types of activity - gaming according to appendix 3-8 to this order;

11) form the notification on the beginning or the termination of activities as the taxpayer performing separate types of activity - electronic trading by goods according to appendix 3-10 to this order;

12) form of the notification on the beginning or the termination of operation of radio-electronic means, according to appendix 3-11 to this order;

13) form of the notification on the beginning or the termination of operation of the high-frequency device according to appendix 3-12 to this order;

14) form of the notification on the beginning or the termination of operation of radio-electronic means for radio amateur services according to appendix 3-13 to this order;

15) form of the notification on the beginning or the termination of operation of radio extension of the telephone channel according to appendix 3-14 to this order;

16) the list of the state bodies performing acceptance of notifications according to appendix 4 to this order;

17) form of the notification on the beginning or the termination of activities as the taxpayer performing production, assembly (picking) of excise goods according to appendix 5 to this order;

18) form of the notification on the beginning or the termination of activities for collection, sorting and (or) transportation of waste, to recovery and (or) destruction of harmless waste according to appendix 6 to this order;

19) form of the declaration on impact on the environment according to appendix 7 to this order.";

in Regulations of Admission of the notifications state bodies approved by the specified order:

state preamble in the following edition:

"These rules are developed according to the subitem 4) of article 11 of the Law of the Republic of Kazakhstan "About permissions and notifications" (further - the Law) and determine procedure for acceptance of notifications by state bodies.";

to state appendices 3-5, 3-10, 3-13, 3-14 and 4 to the order in edition according to appendices 1, of 2, of 3, 4 and 5 to this order;

add with appendices 5, 6 and 7 according to appendices 6, 7 and 8 to this order.

2. To provide to department of policy of development of entrepreneurship in the procedure established by the legislation of the Republic of Kazakhstan:

1) state registration of this order in the Ministry of Justice of the Republic of Kazakhstan;

2) placement of this order on official Internet resource of the Ministry of national economy of the Republic of Kazakhstan;

3) within ten working days after state registration of this order in the Ministry of Justice of the Republic of Kazakhstan submission to Legal department of data on execution of the actions provided by subitems 1) and 2) of this Item.

3. To impose control of execution of this order on the first vice-minister national economies of the Republic of Kazakhstan.

4. This order becomes effective after sixty calendar days after day of its first official publication.
