Item 6 of the Procedure for reference of citizens from among those which participated in liquidation of other nuclear accidents, in nuclear testing, in military exercises using nuclear weapon, in creation of nuclear charges and carrying out on them scheduled works and the victim under other circumstances of radiation exposure not on own fault, to the corresponding categories of persons which were injured owing to the Chernobyl catastrophic crash, the Cabinet of Ministers of Ukraine approved by the resolution of December 2, 1992 No. 674 (ZP of Ukraine, 1992, No. 12, Art. 292), to state in the following edition:
"6. Certification to persons specified in this procedure is performed by Council of Ministers of the Autonomous Republic of Crimea, the regional, Kiev and Sevastopol city state administrations at the registered or actual place of residence.
The decision on refusal in certification can be appealed judicially.
For receipt of the certificate persons submit applications together with the necessary documents specified in items 4 and 5 presents About, district in Kiev and Sevastopol to the state administrations, executive bodies of rural, settlement, city, district councils in the cities and if such service is provided through the center of provision of administrative services - to the centers of provision of administrative services.
Applications together with necessary documents are submitted in paper or, if technically possible - electronically.
