Bring in the resolution of the Cabinet of Ministers of Ukraine of June 29, 2021 No. 677 "Some questions of forming and use of the certificate confirming vaccination against sharp respiratory disease of COVID-19, of SARS-CoV-2, caused by coronavirus negative result of testing or recovery of person from the specified disease" (The Official Bulletin of Ukraine, 2021, No. 54, the Art. 3342, No. 76, the Art. 4771) changes which are applied.

Approved by the Resolution of the Cabinet of Ministers of Ukraine of October 28, 2021 No. 1126
1. To add Item 5 of the resolution after paragraph two with the new paragraph of the following content:
"provide electronic identification and authentication of person, including with use of the integrated system of electronic identification, the digital signature based on the qualified digital signature certificate and/or other means of identification which allow to identify unambiguously the personality during data transmission in the central database of electronic health care system, for the purpose of forming, display and confirmation of action of COVID certificates through the Action Portal, including with use of mobile application of the Action Portal;".
With respect thereto third - the fifth to consider paragraphs respectively paragraphs the fourth - the sixth.
2. According to the procedure of forming and use of the certificate confirming vaccination against sharp respiratory disease of COVID-19, of SARS-CoV-2, caused by coronavirus negative result of testing or recovery of person from the specified disease, approved by the specified resolution:
