1. Bring in the resolution of the Ministry of Agriculture and Food of the Republic of Belarus of May 19, 2017 No. 32 "About establishment of forms of veterinary documents and recognition voided some resolutions of the Ministry of Agriculture and Food of the Republic of Belarus" the following changes:

1. Establish forms:

1.1. veterinary certificates for the purposes of export from customs area of the Eurasian Economic Union of the goods which are subject to veterinary control (supervision):

lethal, breeding and other animals, bird, bees and brood of bees in form 5a according to appendix 1;

fodder grain and other vegetable forages for livestock production in form 5b according to appendix 2;

breeding material (sperm of producers, embryos, incubatory egg, caviar and another) in form 5c according to appendix 3;

milk and dairy products in form 5d according to appendix 4;

meat and meat products of domestic and wild animals in form 5e according to appendix 5;

raw materials of animal origin in form 5f according to appendix 6;

bee honey and products of beekeeping in form 5g according to appendix 7;

fishes, Crustacea, mollusks, water animals, other objects of trade and products of their conversion in form 5h according to appendix 8;

hunting trophies of wild animals, birds, fishes, reptiles, and also their parts and derivatives in form 5i according to appendix 9;

eggs of bird, egg powder and other foodstuff of conversion of egg in form 5j according to appendix 10;

1.2. the veterinary certificates granted by the organizations of the public veterinary service when moving within the territory of the Republic of Belarus and export to countries of the Commonwealth of Independent States, except for state members of the Eurasian Economic Union, which are subject to veterinary control (supervision) of goods:

animal, biological objects in form 1 according to appendix 11;

foodstuff or food staples of animal origin in form 2 according to appendix 12;

technical raw materials of animal origin, forages or feed additives in form 3 according to appendix 13;

1.3. the veterinary certificates granted by specialists in the field of veterinary science of legal entities and individual entrepreneurs based on Items 20 and 21 of the Regulations on procedure and conditions of issue of veterinary documents when moving:

animals in form 1a according to appendix 14;

foodstuff or food staples of animal origin in form 2a according to appendix 15;

technical raw materials of animal origin, forages or feed additives in form 3a according to appendix 16;

1.4. the veterinary and sanitary passport of apiary in form according to appendix 17.";

be reworded as follows appendices 1-5 (are applied);

add the resolution with appendices 6-17 (are applied).

2. This resolution becomes effective after its official publication.
