Due to the adoption of law of Ukraine "About modification of some legal acts of Ukraine concerning purpose of temporary government assistance" of 08.09.2005 N2853-IV I ORDER to children:

1. Approve the application form about purpose of all types of the public assistance which is applied.

2. To department of the state social protection of the population to register this order in the Ministry of Justice of Ukraine.

3. Consider invalid the order of the Ministry of Labour and Social Policy of 22.07.2003 N205 "About Approval of the Application Form about Purpose of All Types of the Public Assistance" registered in the Ministry of Justice 13.08.2003 for N711/8032.

4. To impose control over the implementation of this order on the deputy minister according to distribution of obligations.
