According to Item 3 of the resolution of the Cabinet of Ministers of Ukraine of 17.08.2002 No. 1146 "About enhancement of procedure for granting of the public assistance" and the order of the Cabinet of Ministers of Ukraine of 20.01.2012 No. 2781/0/1-12, for the purpose of increase in system effectiveness of provision of social support to citizens I ORDER:
1. Approve the application form about purpose of all types of the public assistance, compensations, subsidies and privileges which is applied.
2. (V. Mushchinin) to provide to department of the government public assistance of the Ministry of social policy of Ukraine state registration of this order in the Ministry of Justice of Ukraine.
3. Recognize invalid:
3.1. The order of the Ministry of Labour and Social Policy of Ukraine of 08.06.2006 No. 215 "About approval of the application form about purpose of all types of the public assistance", registered in the Ministry of Justice of Ukraine 27.06.2006 for No. 755/12629;
3.2. The order of the Ministry of Labour and Social Policy of Ukraine of 25.08.2009 No. 323 "About modification of the Statement for purpose of all types of the public assistance", registered in the Ministry of Justice of Ukraine 14.09.2009 for No. 869/16885.
4. This order becomes effective since May 1, 2012.
