According to Item 3 of the resolution of the Cabinet of Ministers of Ukraine of August 17, 2002 to No. 1146 "About enhancement of procedure for granting of the public assistance" and to the resolution of the Cabinet of Ministers of Ukraine of February 28, 2015 No. 106 "About enhancement of procedure for provision of housing subsidies", for the purpose of reduction of the regulatory legal act in compliance with requirements of the current legislation I order:

1. Approve the application form about purpose of all types of the public assistance, compensations and privileges which is applied.

2. To provide to department of the government public assistance (Muzychenko V. V.) submission of this order on state registration in the Ministry of Justice of Ukraine.

3. Declare invalid the order of the Ministry of social policy of Ukraine of February 22, 2012 No. 96 "About approval of the application form about purpose of all types of the public assistance, compensations, subsidies and privileges", registered in the Ministry of Justice of Ukraine on March 20, 2012 on No. 418/20731 (with changes).

4. This order becomes effective since May 1, 2015, but not earlier than day of its official publication.
