1. "23. For receipt of the conclusion the organization of health care submits the application to territorial subdivision of state body in the field of rendering medical services (help) (further – Division) in electronic form in management information system resources (further – IS "RMS") in form according to appendix 1 to these rules (further – appendix 1).

24. The organization of health care applying for provision of VTMP in the request specifies data on compliance to criteria to rendering the declared VTMP type (further – criteria) in form according to appendix 2 to these rules.

25. The division carries out assessment of conformity of the organization of health care to criteria within 30 calendar days from the date of receipt of the request.

In case of compliance of the organization of health care to criteria the Division issues the conclusion in form according to appendix 3 to these rules for a period of 3 (three).

No later than 2 (two) months before expiration the organization of health care submits the application for receipt of the new conclusion.

26. The decisions received according to these rules are renewed on the organizations of health care planning and (or) performing activities with preserving production base and profile of activities in cases: reorganization, changes of form of business, change of the legal entity, transfer of object to the trust management confirmed with authorized body or local authorities of public administration by health care.

27. Data on the conclusion are entered in IS "RMS". The conclusion is issued signed by the first division manager and (or) person fulfilling its duties.

28. The organization of health care which received the decision sends the copy of the conclusion to FSMS and (or) its territorial branch.";

"30. In case of change of code or the name VTMP Division based on the request of the organization of health care applying for provision of VTMP in case of compliance to criteria renews the conclusion according to new code or the name within 30 calendar days from the moment of receipt of the request.";

2. To provide to department of the organization of medical care of the Ministry of Health of the Republic of Kazakhstan in the procedure established by the legislation of the Republic of Kazakhstan:

1) state registration of this order in the Ministry of Justice of the Republic of Kazakhstan;

2) placement of this order on Internet resource of the Ministry of Health of the Republic of Kazakhstan after its official publication;

3) within ten working days after state registration of this order in the Ministry of Justice of the Republic of Kazakhstan submission to Legal department of the Ministry of Health of the Republic of Kazakhstan of data on execution of the actions provided by subitems 1) and 2) of this Item.

3. To impose control of execution of this order on the supervising vice-Minister of Health of the Republic of Kazakhstan.

4. This order becomes effective after ten calendar days after day of its first official publication.
